PUBLIC INFORMATION - the information in this section will be made available on the Chamber Website.

Business Name / Individual Name - the name of your business, agency or organization. If an Individual membership, your full name.
Address, City, State and Zip Code - the address you would like the public AND the Chamber to contact you through with any written correspondence.

800 Number, Phone, Fax - Contact numbers made available to the public. If you do not want your Fax number listed on the website, leave that field

Email and Website - To be posted on the website. If you do not want your email address posted on the Chamber website, leave this field empty and complete the private, contact email field below.

Classifications - Select a minimum of one and a maximum of 3 classifications that pertain to your business or organization. You can copy from the complete Classification List, and paste into the form.

Business Description - This information will display on the website when a viewer performs an Advanced Search. Enter a brief description of your business or organization. Include any words or terms that will help the public find you, your products or services.

PRIVATE INFORMATION - the information in this section is for Chamber interoffice use only and will not be displayed on the website.

Membership Type - membership type is determined by the type of business, and number of full time equivalent employees. Full time equivalent is based on the total number of payroll hours per year, divided by 2080.
Contact Name - the individual responsible for payment of membership dues, update of membership information, etc.

Voting Delegate and Alternate Delegate - the individual entitled to vote at Membership meetings, typically the organization's owner, manager or executive director, and a second person authorized to vote as an alternate.

Private Phone, Private Fax, Private Email - contact information for Chamber interoffice use only, if different than the Membership contact information provided.
